Herpes (Cold Sores & Genital Herpes)
Herpes simplex virus (HSV) is a common viral infection that causes recurrent outbreaks of painful blisters on or around the mouth (oral herpes) and the genital area (genital herpes). Many people carry HSV-1 or HSV-2, and infection can be lifelong with periods of activity and remission.
Medical Evaluation and Evidence-Based Treatment
What Is Herpes?
Herpes is caused by the herpes simplex virus (HSV). The two most common types are:
HSV-1
Primarily causes cold sores (oral herpes), but can also affect the genital area.
HSV-2
Most often causes genital herpes.
Both types can be transmitted through direct contact with an infected area, even when sores are not visible.

Symptoms
Herpes symptoms can vary. Some people experience frequent outbreaks; others have mild or no symptoms at all.
Common symptoms include:
Painful blisters or sores on or around the lips, mouth, or genitals
Tingling, itching, or burning before blisters form
Painful urination (genital outbreaks)
Flu-like symptoms during initial outbreak
Outbreaks may recur, especially during times of stress, illness, or immune changes.
Transmission & Prevention
Herpes is most often spread through direct skin-to-skin contact. You can transmit HSV even when no sores are present.
Reducing Transmission Risk Includes:
Daily suppressive antiviral therapy
Avoiding direct contact during outbreaks
Using barrier protection (condoms or dental dams)
Open communication with partners
Suppressive therapy reduces viral shedding and lowers transmission risk but does not eliminate risk entirely.
How It’s Treated
Herpes cannot be cured, but antiviral therapy can:
✔ Reduce severity and duration of outbreaks
✔ Decrease asymptomatic viral shedding
✔ Lower transmission risk when taken consistently
Common antiviral medications prescribed include acyclovir, valacyclovir, and famciclovir when clinically appropriate.
Episodic Therapy
Taken at the first sign of an outbreak.
Suppressive Therapy
Daily antiviral medication to reduce outbreak frequency and reduce risk of transmission.
