Telehealth Information

Effective Date: August 13, 2026

Optima Vida Healthcare, LLC (“Optima Vida Healthcare” or “OVH”) provides healthcare services through telehealth when clinically appropriate and permitted by applicable law.

This page explains generally how telehealth works at Optima Vida Healthcare, its benefits and limitations, and what patients should expect when receiving care remotely.

This information does not replace any telehealth informed consent, privacy notice, or other consent or authorization that may be required before receiving healthcare services.

What Is Telehealth?

Telehealth allows a healthcare professional and patient to communicate and provide healthcare services when they are not physically in the same location.

Depending on the service, telehealth may include:

  • Secure medical questionnaires and intake forms;

  • Review of medical history and current medications;

  • Secure electronic messaging;

  • Telephone communication;

  • Video visits;

  • Review of photographs or other information submitted by the patient;

  • Review and ordering of laboratory testing;

  • Electronic prescribing when clinically appropriate and legally permitted; and

  • Follow-up and ongoing treatment monitoring.

The type of telehealth interaction required depends on the patient's medical needs, the service being requested, clinical judgment, and applicable federal and state requirements.

Where Telehealth Services Are Available

Healthcare services are available only where Optima Vida Healthcare and the treating clinician are legally authorized to provide the applicable service.

You must accurately report your physical location at the time healthcare services are provided.

Service availability may differ depending on:

  • Your physical location;

  • Clinician licensure and prescribing authority;

  • The type of healthcare service;

  • The medication being considered;

  • Federal and state telehealth requirements; and

  • Other applicable laws and regulations.

The fact that information about a particular service appears on our website does not mean that the service is available to every patient or in every state.

How the OVH Telehealth Process Works

Depending on the service and individual circumstances, the telehealth process may include:

  1. Eligibility and location determination
    We determine whether the requested service can be provided in the state where you are physically located.

  2. Identity and patient information
    You may be asked to provide information necessary to establish your identity and create or maintain an appropriate patient record.

  3. Medical intake
    You provide relevant medical history, symptoms, medications, allergies, health conditions, and other information necessary for evaluation.

  4. Clinical review
    A licensed healthcare professional reviews the information provided and determines what additional evaluation may be necessary.

  5. Additional evaluation when required
    Depending on the service, the clinician may require laboratory testing, medical records, photographs, a telephone or video encounter, an in-person examination, or other information.

  6. Independent clinical decision
    The treating clinician independently determines whether a diagnosis can appropriately be made through telehealth and what treatment, if any, is medically appropriate.

  7. Prescription when appropriate
    A prescription is issued only when the treating clinician determines that medication is medically appropriate and prescribing through telehealth is permitted by applicable law.

  8. Follow-up and monitoring
    Follow-up visits, laboratory testing, treatment monitoring, medication adjustments, or referral for other care may be required.

Telehealth Does Not Guarantee a Prescription

Submitting an intake, scheduling an appointment, paying for an evaluation, requesting a medication, or completing a telehealth encounter does not guarantee that a medication will be prescribed.

The purpose of the encounter is to obtain healthcare from a licensed professional, not to purchase a prescription.

Your clinician may determine that:

  • No medication is necessary;

  • A different treatment is more appropriate;

  • Additional information or testing is required;

  • A video or in-person examination is required;

  • Another healthcare professional should evaluate you; or

  • Telehealth is not appropriate for your condition.

Clinical decisions are based on professional judgment, available medical information, applicable standards of care, and legal requirements.

Prescriptions and Pharmacies

When medication is clinically appropriate, prescriptions may be transmitted electronically to an appropriate pharmacy.

The pharmacy is separately responsible for determining whether it can legally and appropriately dispense the prescription.

Medication availability, dispensing, pharmacy processing, shipping, insurance coverage, and pharmacy pricing may vary.

When clinically appropriate and legally permitted, treatment may include an FDA-approved medication or a compounded medication.

Compounded medications are not FDA-approved. The FDA does not review compounded medications for safety, effectiveness, or quality before they are marketed in the same manner as FDA-approved drugs.

Your clinician determines whether a particular treatment is appropriate for you.

Controlled Substances

Controlled substances are subject to additional federal and state prescribing requirements.

Availability through telehealth may be limited by:

  • The patient's physical location;

  • Clinician licensure;

  • State controlled-substance authority;

  • DEA requirements;

  • In-person evaluation requirements; and

  • Other federal or state restrictions.

The availability of a controlled-substance service on our website does not guarantee eligibility or that a controlled substance will be prescribed.

Optima Vida Healthcare may require additional verification, examination, monitoring, laboratory testing, or follow-up for these services.

Benefits of Telehealth

Potential benefits of telehealth include:

  • Improved access to healthcare;

  • Reduced travel;

  • Convenient communication with healthcare professionals;

  • Easier access to follow-up care;

  • Timely review of medical information; and

  • Ability to receive appropriate healthcare from home or another private location.

Limitations and Risks of Telehealth

Telehealth also has limitations and risks.

These may include:

  • A remote examination may provide less information than an in-person physical examination;

  • Technology or internet connections may fail;

  • Images, video, or audio may be inadequate for clinical evaluation;

  • Laboratory testing or an in-person examination may still be necessary;

  • Delays may occur because of technology or communication problems;

  • Electronic communications may involve privacy or security risks;

  • Some medical conditions cannot be safely diagnosed or treated remotely; and

  • A clinician may determine that you need urgent, emergency, specialist, or in-person care.

When the limitations of telehealth prevent an appropriate evaluation, your clinician may recommend another form of care.

Your Responsibilities as a Telehealth Patient

For telehealth to be safe and effective, you should:

  • Provide complete and accurate medical information;

  • Disclose your current medications, supplements, allergies, and relevant medical conditions;

  • Accurately report your physical location;

  • Complete requested laboratory testing or monitoring;

  • Follow treatment and follow-up instructions;

  • Inform your clinician about significant changes in your health;

  • Use a reasonably private environment for video or telephone encounters;

  • Protect your patient portal and account credentials; and

  • Seek in-person or emergency care when instructed.

Do not withhold medical information because you believe it could affect whether a particular medication is prescribed. Your clinician needs accurate information to make safe treatment decisions.

Privacy and Security

Optima Vida Healthcare uses reasonable administrative, technical, and physical safeguards designed to protect patient information.

Our handling of protected health information is described in our Notice of Privacy Practices.

Our practices concerning information collected through our public website and related online services are described in our Privacy Policy.

Patients should use designated secure patient systems when instructed to submit sensitive medical information.

Emergencies

TELEHEALTH SERVICES PROVIDED BY OPTIMA VIDA HEALTHCARE ARE NOT A SUBSTITUTE FOR EMERGENCY MEDICAL CARE.

If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

Do not wait for a response to a website form, patient portal message, email, text message, or asynchronous telehealth request if you may be experiencing an emergency.

In-Person Care and Referrals

Receiving telehealth services from Optima Vida Healthcare does not replace all in-person healthcare.

Depending on your circumstances, you may be advised to obtain care from a primary care clinician, specialist, urgent care center, emergency department, laboratory, imaging facility, or other healthcare provider.

Patients are encouraged to maintain appropriate routine and preventive healthcare in addition to condition-specific telehealth treatment.

Telehealth Consent

Before receiving telehealth healthcare services, you may be required to review and acknowledge a separate telehealth informed consent.

That consent may contain additional information concerning the specific telehealth technologies used, privacy considerations, patient rights, risks, benefits, alternatives, and requirements applicable to your care.

This webpage provides general information and does not replace the telehealth informed-consent process required for patient care.
