There is no charge to complete an intake form. Completing an intake does not establish that treatment is appropriate and does not guarantee a prescription or any specific medication. A licensed healthcare provider will review your information and determine the appropriate next steps.

Testosterone/Phentermine Treatment
Testosterone is a controlled substance. OVH currently considers testosterone and phentermine treatment only for eligible patients receiving care in Oklahoma. Patients outside Oklahoma may still complete a hormone and weight evaluation for other treatment options that do not involve controlled substances.

Controlled-Substance Treatment

1. Complete your intake

Select your service above and complete the secure medical intake. There is no charge to submit the intake form.

2. Create your secure patient account

You will receive an invitation to Practice Better. You must accept this invitation so OVH can securely communicate with you, send forms, and coordinate your care.

3. Complete required forms

Depending on the service, you may be asked to complete OVH's general consent forms and treatment-specific informed consent.

4. Complete payment and any required testing

Applicable evaluation fees will be clearly displayed before payment. Some services may require laboratory testing before treatment can be considered.

5. Provider evaluation

Your provider will review your medical history, symptoms, medications, laboratory results when applicable, and other relevant information. A video or in-person evaluation may be required depending on the service, clinical circumstances, or applicable law.

6. Treatment decision

Your provider will independently determine whether treatment is medically appropriate. If medication is appropriate, treatment options, risks, alternatives, monitoring, and pharmacy options can be discussed.

What Happens Next?

Completing an intake, paying an evaluation fee, or requesting a particular treatment does not guarantee that a prescription will be issued.

If you are already an established patient, you generally do not need to complete another new-patient intake.
